Resumen Introducción: Las lesiones de la mano son uno de los padecimientos más frecuentemente atendidos en el Servicio de Cirugía Plástica y Reconstructiva del Hospital Central Militar, dentro de la amplia variedad de estas lesiones se encuentran las lesiones del sistema flexor. Objetivo: Estimar la incidencia, funcionalidad y tiempo de reintegración laboral en pacientes con lesiones de sistema flexor de la mano tratados en el Hospital Central Militar en el periodo del 1º de marzo del 2020 al 1º de marzo del 2022. Metodología: Estudio observacional, retrospectivo, longitudinal descriptivo en pacientes adultos con lesión de sistema flexor de la mano, en los cuales la causa de la lesión no haya condicionado un trauma complejo de la mano. Se utilizó como referencia los criterios de recuperación funcional de Strickland y Goglovac. Resultados: A pesar de que las lesiones de la zona II flexora en la literatura presentan un peor pronóstico para la recuperación de la función normal de la mano, los resultados demuestran que la mayor parte de los pacientes se encuentra en una escala funcional buena. Limitaciones del estudio: El estudio fue llevado a cabo durante el periodo comprendido por la pandemia COVID, por lo que las consultas subsecuentes con tiempos más alargados y el inicio tardío de las consultas de rehabilitación pudieron haber contribuido hacia una recuperación más deficiente de la recuperación funcional de los pacientes.


Abstract Introduction: Hand injuries are one of the conditions most frequently treated in the Plastic and Reconstructive Surgery Service of the Hospital Central Militar, within the wide variety of these injuries are injuries to the flexor system. Objective: To estimate the incidence, functionality and labor reintegration time in patients with hand flexor system injuries treated at the Hospital Central Militar in the period from March 1, 2020 to March 1, 2022. Methodology: Observational, retrospective, longitudinal, descriptive study in adult patients with injury to the flexor system of the hand, in whom the cause of the injury did not result in complex trauma to the hand. The Strickland and Goglovac functional recovery criteria were used as reference. Results: Despite the fact that flexor zone II lesions in the literature present a worse prognosis for the recovery of normal hand function, the results show that most of the patients are in a good functional scale. Limitations of the study: The study was carried out during the period covered by the COVID pandemic, so that subsequent consultations with longer times and the late start of rehabilitation consultations could have contributed to a poorer recovery of functional recovery. from the patients.

Introdução: cerca de 5% da população brasileira pratica corrida de rua, e a falta de preparo físico das pessoas para esse esporte contribui para o aumento no índice de lesões, sendo o joelho uma das articulações mais acometidas. Sabendo que os parâmetros musculares interferem diretamente no desempenho e que a destreza de membros pode suscitar inferências relativas ao aumento de força no membro dominante, torna-se importante avaliar a força e a relação de equilíbrio muscular do joelho destes atletas tanto no membro dominante quanto no contralateral. Objetivo: verificar o equilíbrio muscular dos extensores e flexores de joelho dominante e não dominante em corredores recreacionais. Metodologia: foram incluídos 111 indivíduos com idade entre 18 e 65 anos, de ambos os sexos, praticantes de corrida há pelo menos 4 meses contínuos, sem histórico de lesão nos últimos 3 meses. Os participantes foram entrevistados e encaminhados para a coleta da força muscular isocinética dos grupos extensor e flexor do joelho com protocolo de 60°/s, 180°/s e 300°/s. As variáveis de interesse estudadas foram: membro dominante, tempo de prática de corrida, torque máximo, trabalho total e potência. Foram calculados os Índices de Deficiência Muscular (IDM) sendo admitido como referencial de equilíbrio até 10% na diferença entre os membros. A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa do Instituto de Ciências da Saúde da Universidade Federal da Bahia, sob parecer de nº 2.621.166. Resultados: o IDM indicou que 54,1% dos extensores de joelho estavam equilibrados e que 55,9% dos flexores de joelho encontravam-se numa relação de desequilíbrio muscular. Conclusão: a dominância de membros não é um fator ligado ao desequilíbrio de forças no membro inferior. Este achado de desequilíbrio de força entre joelhos pode estar ligado à predisposição de lesão e deve orientar as equipes multiprofissionais de saúde a definir um trabalho preventivo de treinamento muscular e esportivo.


Introduction: about 5% of the Brazilian population practices street running and the lack of physical fitness for this sport contributes to the increase in the injury rate, with the knee being one of the most affected joints. Knowing that muscle parameters directly interfere performance and that limb dexterity may lead to inferences regarding strength increase in the dominant limb, it is important to evaluate the strength and balance ratio of these athletes' knee in both dominant and contralateral limbs. Objective: to verify muscle balance of dominant and non-dominant knee extensors and flexors in recreational runners. Methodology: 111 individuals aged between 18 and 65 years old, male and female, who have been running for at least 4 continuous months, with no history of injury in the last 3 months, were included. Participants were interviewed and referred for the collection of isokinetic muscle strength of the knee extensor and flexor groups with a protocol of 60°/s, 180°/s and 300°/s. The variables of interest studied were: dominant limb, running practice time, maximum torque, total work and power. Muscle deficiency indices (MDI) were calculated and accepted as a balance reference up to 10% in the difference between the limbs. The research was approved by the Research Ethics Committee of the Institute of Health Sciences of the Federal University of Bahia, under technical advice no 2.621.166. Results: the MDI indicated that 54.1% of the knee extensors were balanced and 55.9% of the knee flexors were in a muscular imbalance ratio. Conclusion: limb dominance is not a factor related to lower limb imbalance of forces. This finding of strength imbalance between knees may be associated to injury predisposition and should guide multidisciplinary health teams to define preventive work on muscle and sports training

Abstract Objective: To evaluate the contamination rate of hamstring tendon autografts by comparing two different techniques, and to verify whether intraoperative contamination is associated with the development of clinical infection in patients submitted to reconstruction of the anterior cruciate ligament (ACL). Methods: A total of 110 hamstring tendon autograft ACL reconstructions were performed and divided into two groups: 1-hamstring tendon retraction technique; and 2 - technique maintaining the tibial insertion of the hamstring tendon. During the preparation, two graft fragments were sent for culturing; the harvesting time, the preparation time, and the total surgery time were measured. Twenty-four hours after the surgery, the C-reactive protein was assayed. The clinical outpatient follow-up was performed up to 180 days postoperatively. Results: Although there were two postoperative infections, there was no graft contamination or difference between the groups in relation to the graft preparation time and to the 24-hour postoperative C-reactive protein assessment. The classic technique presented a longer graft harvesting time (p = 0.038), and there was no statistical difference between the 2 groups regarding the degree of contamination and consequent clinical infection, although 2 patients in group 2 presented with infection, with negative perioperative cultures. Conclusion: Based on the results obtained, there was no association between graft contamination and the time or technique of its preparation. In addition, there was also no association between intraoperative contamination and the development of clinical infection, nor was there any sign of an association between the early alteration of Creactive protein and the onset of infection.


Resumo Objetivo: Avaliar a taxa de contaminação de autoenxerto de tendões flexores comparando duas técnicas e verificar se a contaminação intraoperatória está associada ao desenvolvimento de infecção clínica em pacientes submetidos a reconstrução do ligamento cruzado anterior. Métodos: Foram feitas 110 reconstruções do ligamento cruzado anterior com tendão dos flexores e divididas em dois grupos: 1) técnica com retirada total dos tendões flexores e 2) técnica que manteve a inserção tibial desses tendões. Durante o preparo, dois fragmentos de cada um desses foram enviados para cultura, sendo mensurado o tempo de retirada dos tendões, do preparo dos tendões e total da cirurgia. Com 24 horas de pós-operatório, foi dosada a proteína C reativa. Seguimento clínico ambulatorial foi realizado de forma protocolada até 180 dias de pós-operatório. Resultados: Apesar de terem sido observadas duas infecções pós-operatórias, não houve contaminação dos enxertos nem diferença entre os grupos emrelação ao tempo de preparo dos enxertos e a proteína C reativa com 24 horas de pós-operatório. A técnica clássica apresentou maior tempo de retirada do enxerto (p = 0,038) e não houve diferença estatística entre os dois grupos no que tange ao grau de contaminação e consequente infecção clínica, embora dois pacientes do grupo 2 tenham tido infecção com culturas perioperatórias negativas. Conclusão: Com base nos resultados obtidos, não houve associação entre contaminação do enxerto com o tempo ou a técnica de sua preparação, tampouco entre a contaminação intraoperatória e o desenvolvimento de infecção clínica ou entre alteração precoce da proteína C reativa e o surgimento de infecção.

ABSTRACT

La tenosinovitis con cuerpos libres en forma de granos de arroz es una afección muy infrecuente que se presenta, generalmente, asociada a artritis reumatoide o tuberculosis. Los casos no asociados a estas enfermedades son extremadamente raros. Se presenta un paciente masculino de 62 años de edad, intervenido quirúrgicamente por un síndrome del túnel carpiano derecho hace 7 años. En aquel momento se le realizó una apertura del retináculo transverso del carpo y una sinovectomía de los tendones flexores. La biopsia sinovial reportó una "sinovitis crónica". Evolucionó satisfactoriamente durante los primeros 4 años, luego comenzó a presentar nuevamente los síntomas compresivos del nervio mediano y un aumento progresivo del volumen en la cara anterior de la muñeca. El ultrasonido mostró una marcada hipertrofia sinovial de los tendones flexores a nivel de la muñeca. Los estudios neurofisiológicos reportaron daño axonal y mielínico en las fibras del nervio mediano derecho. Se le realizó una nueva intervención donde se visualizó la hipertrofia sinovial y abundantes cuerpos libres en forma de granos de arroz agrupados en sacos sinoviales. Se le descomprimió el túnel carpiano, se resecaron los cuerpos libres y se le hizo una sinovectomía amplia. El resultado de la biopsia fue una sinovitis inespecífica con cuerpos libres en granos de arroz. Después de dos años de operado el paciente está asintomático.


Tenosynovitis with loose rice grain-like structures is a very rare condition that usually presents associated with rheumatoid arthritis or tuberculosis. Cases not associated with these diseases are extremely rare. A 62-year-old male patient underwent surgery for a right carpal tunnel syndrome seven years ago. At that time, an opening of the transverse retinaculum of the carpus and synovectomy of the flexor tendons were performed. Synovial biopsy reported "chronic synovitis". It evolved satisfactorily during the first four years; then the patient began to present again the compressive symptoms of the median nerve and progressive increase in volume in the anterior aspect of the wrist. The ultrasound showed marked synovial hypertrophy of the flexor tendons at the level of the wrist. Neurophysiological studies reported axonal and myelin damage in the fibers of the right median nerve. A new intervention was performed. Synovial hypertrophy was visualized and abundant free loose rice grain-like structures grouped in synovial sacs. The carpal tunnel was decompressed, the free bodies were resected and wide synovectomy was performed. The result of the biopsy was non-specific synovitis with free rice grain-like structures. After two years of surgery, the patient is asymptomatic.


La ténosynovite à corps étrangers libres d'aspect en grains de riz est une affection très peu fréquente, étant généralement associée à l'arthrite rhumatoïde ou à la tuberculose. Les cas non associés à ces maladies sont extrêmement rares. Le cas d'un patient âgé de 62 ans, atteint de syndrome du canal carpien droit, et traité chirurgicalement il y a sept ans, est présenté. En ce moment-là, il a subi une incision du ligament transverse du carpe (rétinaculum) et une synovectomie des tendons fléchisseurs. La biopsie synoviale a dévoilé une «synovite chronique¼. L'évolution a été satisfaisante au cours des quatre premières années; puis après la compression du nerf médian et l'inflammation progressive de la face antérieure du poignet ont réapparues. L'échographie a montré une marquée hypertrophie synoviale des tendons fléchisseurs au niveau du poignet. Les examens neurophysiologiques ont révélé un dommage axonal et myélinique des fibres du nerf médian droit. Dans la nouvelle intervention effectuée, une hypertrophie synoviale et plusieurs corps étrangers d'aspect en grains de riz groupés en bourses synoviales ont été remarqués. Une décompression du canal carpien, une résection des corps étrangers et une large synovectomie ont été réalisées. La biopsie a repéré une synovite non spécifique à corps étrangers libres en forme de grains de riz. Le patient reste encore asymptomatique depuis deux ans.

ABSTRACT

Los vínculos son estructuras especializadas inherentes a los tendones flexores de los dedos, y tienen gran importancia en la nutrición de los mismos, además de facilitar la flexión de los dedos después de una transección distal del tendón (es), permitiendo que éste (os) actuen indirectamente a través de las articulaciones interfalángicas. El presente estudio tuvo como objetivo revisar aspectos morfofuncionales de los vínculos largos y cortos de los músculos flexores de los dedos de la mano. El trabajo fue realizado en 60 manos de cadáveres formolizados de individuos adultos, brasileños, con una edad promedio de 53,1±17,8 años. Los vínculos cortos fueron encontrados en 100 % de los casos, tanto en los músculos flexores superficiales de los dedos como en los profundos, siempre en número de uno en cada tendón y su localización siempre estuvo en la extremidad del tendón, antes de la inserción de éste en la articulación interfalángica proximal. Los vínculos largos presentaron gran variación morfológica, con una presencia promedio de 61,3 % en los tendones del músculo flexor superficial y de 77,9 % en el músculo flexor profundo. El número de vínculos por tendón varió aún más, no siendo posible identificar un patrón independientemente del músculo. En relación a la localización de los vínculos largos, en los tendones del músculo flexor superficial de los dedos, están insertos en la vaina fibrosa de la falange proximal, con un trayecto oblicuo y próximo a la articulación interfalángica proximal; los vínculos de los tendones del músculo flexor profundo estaban localizados en su mayoría (86,7 %) entre los tendones a nivel de la articulación interfalángica proximal, presentando un trayecto rectilíneo entre esos dos tendones, mientras que en 13,3 % estuvieron localizados muy próximos a la abertura del tendón del músculo flexor superficial para el paso del tendón del músculo flexor profundo. Los vínculos son estructuras escenciales en la mantención de la nutrición y de los movimientos de los dedos aún en presencia de lesiones, por lo tanto, es importante tener una comprensión clara de ellos para una correcta evaluación clínica pensando en la reparación, reconstrucción y rehabilitación de las disfunciones de la mano.


The vincula are specialized structures inherent in the flexor tendons of the fingers, and are of great importance in their nutrition. In addition they facilitate flexion of the fingers following a distal transection of this tendon, allowing it to operate indirectly across the interphalangeal joints. The aim of the present study was to describe anatomic and biometric aspects of the long and short vincula of the flexor muscles of the fingers. The work was carried out in hands of 60 adult Brazilian corpses, of both sexes, with an average age between 17.8±53.1 years. The short vincula were found in 100 % of cases of flexor digitorum superficialis tendons and flexor digitorum profundus muscles, and only one in each tendon, with its location always at the extremity of the tendon, prior to insertion in the proximal interphalangeal joint. The long vincula presented great morphological variation, with an average presence of 61.3 % in tendons of the flexor digitorum superficialis and 77.9 % in the flexor digitorum profundus muscle. Furthermore, the number of vincula per tendon changed, and it was not possible to identify a pattern independently of the muscle. With respect to the location of the long vincula, in the flexor superficial tendons they are inserted in the fibrous sheath of the proximal phalanx, with an oblique course and next to the proximal interphalangeal joint. The vincula of the tendons of the flexor digitorum profundus were located (86.7 %) between both tendons to level of the proximal interphalangeal joint, presenting a rectilinear course between these two tendons, whereas in 13.3 % they were located very close to the opening of the flexor superficialis muscle tendon for passage of the flexor digitorum profundus muscle tendon. The vincula are essential structures in nutrition and finger motion even when injuries are present. It is therefore, important to have a clear understanding of these structures forproper clinical evaluation in the repair, reconstruction and rehabilitation of hand dysfunctions.

ABSTRACT

Avaliar se a retirada dos dois tendões flexores (semitendíneo [ST] e grácil [GC]) tem o mesmo índice de lesão nervosa que a retirada isolada do tendão ST usado como enxerto triplo. Métodos: Foi avaliada a alteração de sensibilidade relacionada à lesão do ramo infrapatelar do nervo safeno em 110 pacientes seis meses após serem submetidos à reconstrução do LCA com o uso dos tendões flexores, dividido num grupo no qual se usou somente o ST e outro com o ST e o GC. Resultados: O grupo no qual se usou somente o ST como enxerto apresentou um índice de lesão nervosa de 36,1% e no grupo com os tendões ST e GC 58,1% dos pacientes tiveram alteração da sensibilidade. Na avaliacão geral de todos os pacientes o índice de lesão nervosa foi de 50,9%. Conclusão: A retirada do ST isolado e usado de forma tripla é uma opção viável na reconstrução do LCA e pode ocasionar um menor número de lesão nervosa relacionada a ramos do nervo safeno.


The aim of this study was to assess whether harvesting of two hamstring tendons (semitendinosus and gracilis) has the same rate of nerve injury as harvesting of the semitendinosus tendon alone, used as a triple graft. METHODS: Changes in sensitivity relating to injury of the infrapatellar branch of the saphenous nerve were evaluated in 110 patients six months after they underwent anterior cruciate ligament (ACL) reconstruction using hamstring tendons. They were divided into two groups: one in which only the semitendinosus was used and the other, the semitendinosus and gracilis. RESULTS: The group in which only the semitendinosus was used as a graft presented a nerve injury rate of 36.1%. In the group in which the semitendinosus and gracilis tendons were used, 58.1% of the patients presented altered sensitivity. In the general assessment on all the patients, the nerve injury rate was 50.9%. CONCLUSION: Harvesting the semitendinosus alone and using it in triple form is a viable option for ACL reconstruction and may give rise to fewer nerve injuries relating to branches of the saphenous nerve.

ABSTRACT

El presente trabajo se propone comparar los diámetros de los vientres del músculo bíceps braquial y músculo braquial con el objeto de esclarecer algunas dudas sobre cuál sería el músculo más potente en la flexión del antebrazo. Las comparaciones fueron realizadas en 50 miembros superiores de cadáveres fijados. Se hizo evidente la diferencia promedio de 0,51 cm (5,8 por ciento) en el diámetro del vientre muscular del músculo braquial, ratificando lo encontrado en la revisión bibliográfica, donde el músculo braquial es un importante flexor del codo.


The present work seeks to compare the diameter of the wombs of the muscle brachial biceps and m. brachial in the intention of settling the doubts on which would be the most potent muscle in the flexing of the forearm. The measures were accomplished in 50 superior members of fastened corpses. A medium difference of 0.51 cm (5.8 percent) was evident in the diameter of the muscular womb the largest for m. brachial, ratifying what was found in the bibliographical revision, that brachial muscle is an important flexor of the elbow.

ABSTRACT

La rehabilitación de las lesiones de los tendones flexores de los dedos de la mano es uno de los grandes retos que enfrentan los fisioterapeutas por lo impredecible de los resultados de su inter-vención y por el gran número de técnicas quirúrgicas y de rehabilitación que se utilizan, señal de la importancia del tema. El objetivo de la investigación es identificar estudios e investigaciones sobre los resultados de los procesos de rehabilitación fisioterapéutica después de la reparación quirúrgica de los tendones flexores. Se incluyen estudios sobre la reparación quirúrgica y re-habilitación de tendones flexores en cualquier zona de la mano, la búsqueda se realiza en bases de datos utilizando palabras claves como tenorrafia, tendones flexores y rehabilitación. En los estudios revisados no hay unanimidad en los resultados encontrados en cuanto a los protocolos de movilización activa y/o pasiva, tiempo de inicio y la prolongación de los mismos. Los datos encontrados no son concluyentes sobre que técnica de movilización arroja los mejores resulta-dos. Como hallazgo importante para la promoción de hábitos saludables, se encontró que el ta-baquismo influía como factor negativo en la reparación del tendón flexor, al igual que la lesión concomitante del nervio. Como técnica nueva de intervención se utilizó la representación mental de movimientos limitados, nuevo enfoque que necesita más investigación.


Rehabilitation of flexor tendon injuries of the fingers, is one of the greatest challenges faced by physiothe-rapists due to the unpredictability of the action results and the large number of surgical and rehabilitation techniques that are used, signal of the topic importance. The objective of this research is to identify studies and researches on the results in physiotherapy rehabilitation process after surgical repair of flexor tendons. Some studies are included on the surgical repair and rehabilitation of flexor tendons in any area of the hand, the search was done in databases using keywords like tenorrhaphy, flexor tendons and rehabilita-tion. In the reviewed studies there is no unanimity in the findings in terms of active mobilization protocols and / or passive, start time and the extension thereof. The data found are inconclusive on mobilization technique that yields the best results. As important finding for the promotion of healthy habits, it was found that smoking influenced as a negative factor in the flexor tendon repair, as well as concomitant nerve injury. As new intervention technique, it was used the mental representation of limited movement, which needs to be delved

ABSTRACT

The digital flexor muscles of the thoracic limb of four llamas were dissected and illustrated to provide data about the suspensory (support) apparatus and weight bearing structures. An extensive literature search was performed and yielded incomplete information about these anatomical structures. The popularity of the llama world wide as a domesticated animal used for show and fiber has increased in the recent years. It is helpful to describe the anatomy fully to aid in understanding of the species and treatment of pathologic conditions. The description of the anatomical structures and the original illustrations demonstrate genuine peculiarities and differences between the llama and domestic ruminants. In llamas, the three heads of the deep digital flexor muscle (DDF) originate and connect with each other in a very peculiar combination, with one tendon receiving an attachment from the flexor carpi ulnaris muscle (FCU). The superficial digital flexor muscle (SDF) has a thin tendon which is fused with the palmar fascia and then broadens. There are no interflexor muscles. Additionally, unexpected lumbricalis muscles are found in the distal limb and vary in number between the fore and hind limb. The anatomy of the suspensory apparatus in the thoracic limb is evaluated in this paper.


Fueron disecados e ilustrados los músculos flexores digitales del miembro torácico de cuatro llamas con la finalidad de aportar datos sobre el aparato suspensor (de apoyo) que soportan estas estructuras. Se realizó una extensa búsqueda bibliográfica dada la incompleta información existente de estas estructuras anatómicas. La popularidad de la llama en todo el mundo como uno de los animales domésticos utilizados ha aumentado en los últimos años. Es importante describir detalladamente la anatomía de esta especie para su tratamiento en condiciones patológicas. La descripción de las estructuras anatómicas y las ilustraciones originales demuestran peculariedades genuinas y diferencias entre la llama y los rumiantes domésticos. En llamas, las tres cabezas del músculo flexor profundo de los dedos se originan y se conectan entre ellas en una muy peculiar combinación, con uno de los tendones recibiendo un fascículo del músculo flexor ulnar del carpo. El músculo flexor superficial de los dedos tiene un tendón delgado el cual se fusiona con la fascia palmar y luego se amplía. No existen músculos interflexores. Adicionalmente, fueron encontrados músculos lumbricales en la extremidad distal y variaban en número entre los miembros. Finalmente, en este trabajo se evalúa el aparato suspensorio del miembro torácico.

ABSTRACT

This paper provides data (text and illustrations) about the digital flexor muscles of the pelvic limb and the / metatarsophalangeal joint, evaluating the suspensory (support) apparatus and weight bearing structures. Similar to the above mentioned paper, a literature search provided incomplete information about these anatomical structures. As in the thoracic limb, unique anatomically variations exist in the pelvic limb of the llama. The caudal tibial muscle is fused with the lateral head of the deep digital flexor muscle (DDF), and the soleus muscle is missing. A symmetrical unexpected lumbricalis pedis muscle was found; the tendons are fusing with the axial branches of the lateral tendon of the long digital extensor muscle. A quadratus plantae muscle, also unexpected is present on the medial aspect of the tarsal region. The superficial digital flexor muscle (SDF) resembles that of the domestic ruminants. The metacarpo/ metatarsophalangeal joints, referred to as the fetlock joints (FJ) are very different from those of the domestic ruminants. Particular structures were found and they will be described and illustrated in the text. The anatomy of the suspensory (support) apparatus in the pelvic limb is evaluated in this paper.


El presente artículo ofrece datos (texto e ilustraciones) sobre el músculo flexor digital del miembro pélvico y la articulación metatarsofalángica, evaluando el aparato suspensorio (de apoyo) y el peso que soportan estas estructuras. La literatura proporciona información incompleta acerca de estas estructuras anatómicas. Al igual que en el miembro torácico, existen variaciones anatómicamente únicas en el miembro pélvico de la llama. El músculo tibial caudal se fusiona con la cabeza lateral del músculo flexor digital profundo (FDP), y el músculo soleo no existía. Fue encontrada una simetría inesperada del músculo lumbrical del pie; los tendones se encontraban fusionados con las ramas axiales del tendón lateral del músculo extensor digital largo. Un músculo cuadrado plantar, se encontraba presente en la cara medial de la región tarsal. El músculo flexor digital superficial (FDS) se asemeja al de rumiantes domésticos. Las articulaciones metacarpo/metatarsofalángicas, denominadas articulaciones del nudillo (AN) son muy diferentes de las de rumiantes domésticos. Fueron encontradas estructuras particulares que se describen e ilustran en el texto. Se evalúa la anatomía del aparato suspensorio (de apoyo) en el miembro pélvico.

ABSTRACT

A lot has been reported about variation of palmaris longus muscle in journals and conventional textbooks. The incidence of palmaris longus muscle absence among people of the six geo-political zones of Nigeria was evaluated using 600 subjects. 31.25 percent (187 subjects) lack palmaris longus muscleon either of the forearms. 12.5 percent (75 subjects) shows unilateral absence and 18.75 percent (112 subjects) shows bilateral absence. 25 percent (150) of subjects lacks palmaris longus muscleon their right forearms while 25 percent also lacks it on their left forearms. 6 percent (37) shows double tendons of palmaris longus muscleon both forearms. Highest incidence of absence was recorded among people of South-West zone.


Mucho se ha informado acerca de la variación del músculo palmar largo en revistas y libros de texto convencionales. La incidencia de ausencia del músculo palmar largo entre las personas de las seis zonas geo-políticas de Nigeria se evaluó usando 600 sujetos. 31,25 por ciento (187 sujetos) no tenían músculo palmar largo en cualquiera de los antebrazos, 12,5 por ciento (75 sujetos) mostró ausencia unilateral y 18,75 por ciento (112 sujetos) mostró ausencia bilateral. 25 por ciento (150) de los sujetos carece de músculo palmar largo en su antebrazo derecho, mientras que el 25 por ciento carece de él también en su antebrazo izquierdo. 6 por ciento (37) muestra dobles tendones de músculo palmar largo en ambos antebrazos. Mayor incidencia de ausencia se registró entre las personas de la zona Sur-Oeste.

ABSTRACT

OBJETIVO: Avaliar a amplitude de movimento (ADM) e o torque flexor e extensor do joelho após a realização de um programa de alongamento ativo excêntrico dos músculos flexores do joelho. MATERIAIS E MÉTODOS: Trinta e quatro voluntários (23 mulheres e 11 homens), 34,42±9,3 anos, realizaram um programa de alongamento ativo excêntrico dos músculos flexores do joelho na postura em pé, que consistiu de sete repetições de um minuto com 30 segundos de descanso entre as repetições. O programa de alongamento foi realizado duas vezes por semana, durante quatro semanas. A ADM de extensão e o torque flexor e extensor do joelho foram avaliados no dinamômetro isocinético pré e pós-programa de alongamento. O torque foi avaliado nos modos isométrico e isocinético concêntrico e excêntrico a 30º/s e 60º/s. RESULTADOS: Houve aumento na ADM de 53,7±13º para 30,1±16º (p=0,0001), no torque isométrico flexor de 89±32Nm para 93±33Nm (p=0,01) e extensor de 178±67Nm para 187±73Nm (p=0,006). O torque flexor concêntrico e excêntrico a 30º/s aumentou de 90±31Nm para 96±31Nm (p=0,001) e de 100±34Nm para 105±35Nm (p=0,01), respectivamente. O torque extensor concêntrico a 60º/s aumentou de 144±51Nm para 151±58Nm (p=0,02) e o excêntrico a 30º/s de 175±71Nm para 189±73Nm (p=0,01). CONCLUSÕES: O programa de alongamento proposto foi efetivo para aumentar a flexibilidade dos músculos alongados e torque dos grupos musculares agonistas (alongados) e seus antagonistas.


OBJECTIVE: To evaluate the changes in knee range of motion (ROM) and torque of knee flexor and extensor muscles after an active eccentric stretching program for the knee flexor muscles. METHODS: Thirty-four volunteers (23 women and 11 men), aged 34.42±9.3 years, performed an active eccentric stretching program for the knee flexor muscles in the standing posture, consisting of seven repetitions of one minute each, with 30 seconds of resting between them. The stretching program was performed twice a week, for four weeks. Knee extension ROM and the torque of the knee flexor and extensor muscles were evaluated using an isokinetic dynamometer before and after the stretching program. The torque was evaluated in the isometric, isokinetic concentric and eccentric modes at 30º/s and 60º/s. RESULTS: There was an increase in ROM from 53.7±13º to 30.1±16º (p=0.0001), in isometric torque of the flexors from 89±32Nm to 93±33Nm (p=0.01) and of the extensors from 178±67Nm to 187±73Nm (p=0.006). The concentric and eccentric torque of the flexors at 30º/s increased from 90±31Nm to 96±31Nm (p=0.001) and from 100±34Nm to 105±35Nm (p=0.01), respectively. The concentric torque of the extensors at 60º/s increased from 144±51Nm to 151±58Nm (p=0.02), and the eccentric torque at 30º/s increased from 175±71Nm to 189±73Nm (p=0.01). CONCLUSIONS: The stretching program proposed was effective for increasing the flexibility of the stretched muscles and the torque of the agonist (stretched) muscle groups and their antagonists.

ABSTRACT

OBJETIVO: Verificar a amplitude de movimento (ADM) em mãos que sofreram reparo tendinoso dos músculos flexores superficial e profundo dos dedos, comparando os dados de cada dedo na mão lesada e entre mãos lesadas e não lesadas. MÉTODOS: Foi realizada a goniometria ativa em 15 pacientes e 120 dedos, 60 dedos de mãos lesadas e 60 de mãos controle não lesadas. Os sujeitos foram avaliados no momento da retirada da tala gessada, tendo sido realizada a movimentação precoce pelo método de Duran modificado. A partir dos dados goniométricos, foram registrados os valores do índice TAM (Total Active Motion) dos dedos nas mãos lesadas e controle. Para análise dos dados, foi acessada a fórmula de índices funcionais proposta pela American Society for Surgery of the Hand (ASSH) e para cálculo estatístico, foi escolhido o Modelo de Efeitos Mistos. RESULTADOS: A fórmula da ASSH para os dedos lesados mostrou que 18,33 por cento tiveram a classificação do movimento "bom", 18,33 por cento, "regular" e 63,34 por cento, "pobre". Foram comparadas as médias das medidas em graus de todos os dedos entre si dentro de cada grupo, controle ou lesado, e as médias das medidas entre os grupos, encontrando-se um p-valor significante apenas entre os grupos controle e lesado. Não houve diferença estatística entre o TAM de cada dedo na mão lesada. CONCLUSÃO: Independente de quantos dedos tenham sofrido lesão tendinosa em uma mão, os dedos não lesados também terão suas ADMs ativas diminuídas no período logo após a retirada da imobilização.


OBJECTIVE: To assess the range of motion (ROM) in hands that underwent tendon repair in the flexor digitorum superficialis and flexor digitorum profundus muscles of the fingers, comparing the data between the fingers on the injured hand, and between the injured and uninjured hands. METHOD: Active goniometry was performed on 15 patients, making a total of 120 fingers (60 on injured hands and 60 on noninjured control hands). The patients were examined at the time of removing the plaster splint. Early mobilization was performed using the modified Duran method. Goniometric data were used for recording the TAM (total active motion) values of the fingers on the injured and uninjured (control) hands. To analyze the data, the functional index formula proposed by the American Society for Surgery of the Hand (ASSH) was utilized, and for statistical calculations the mixed-effect model was selected. RESULTS: The ASSH formula for the injured fingers classified the movement as "good" in 18.33 percent, "fair" in 18.33 percent and "poor" in 63.34 percent. The means, in degrees, of the measurements for all the fingers were compared with each other within each group (control and injured) and between the groups. A significant difference was found between control and injured groups. There was no statistical difference between the TAM of each finger on the injured hand. CONCLUSION: Independent of how many fingers on one hand had suffered tendon injuries, the uninjured fingers also presented diminished active ROM during the period immediately after removal of the immobilization.

Através dos estudos eletromiográficos é que se tem conhecido a efetiva participação dos músculos esqueléticos. Participaram do estudo 05 sujeitos do sexo masculino, na faixa etária entre 17 e 22 anos, praticantes de musculação. Foram utilizados eletrodos de superfície. Os músculos reto femoral, vasto lateral e vasto medial foram analisados no movimento de extensão do joelho realizado sentado na mesa romana; os músculos bíceps femoral, semimembranoso e semitendinoso foram analisados no movimento de flexão do joelho em decúbito ventral na mesa romana. Os resultados expressos em RMS foram: 543,86 (±127,39) para o músculo semimembranoso, 400,08 (±124,10) para o semitendinoso, 222,92 (±69,28) o valor do bíceps femoral, 288,12 (±152,4) para o músculo reto femoral, 215,63 (±117,30) para o músculo vasto lateral e 190,58 (±75,01) o valor do vasto medial. As cargas máximas utilizadas foram maiores no movimento de extensão do joelho. Os resultados demonstraram maior participação dos músculos flexores do joelho quando coparado com os extensores.


Through the electromyography studies it is that is had known the effective participation of the skeletal muscle. They participated in the study five subject of the masculine sex, in the age group among 17 and 22 years. Surface electrodes were used. The vastus lateralis, vastus medialis, rectus femoris muscles was analyzed in the extension movement of the knee, sat down in the roman table; the biceps femoris, semimembranosus and semitendinosus muscles was analyzed in the flexion movement of the knee, in decubitus ventralis, in the roman table. The results, expressed in RMS, were: 543,86 (±127,39) for the semimembranosus muscle, 400,08 (±124,10) for semitendinosus, 222,92 (±69,28) the value of the biceps femoris muscles, 288,12 (±152,4) for the rectus femoris, 215,63 (±117,30) for the vastus lateralis muscle and 190,58 (±75,01) the value of vastus medialis. The used maximum loads were larger in the extension movement of the knee. The results demonstrated larger participation of the flexors muscles of he knee when compared with the extensors muscles.
